1 adult 2 teenage kids the adult is not doing well. Not sure what happened.

From councilman Sal DiCiccio, a Phoenix firefighter died from injuries sustained during the rescue attempt.

Seems like the firefighter was actually one of the climbers who fell. He was off-duty, out climbing with two friends.

Maybe the anchor suddenly came loose? Any climbers with ideas?

This is so sad to hear. My guess from the videos is that it was probably at the spot known as "rescue rock" where there used to be one enormous bomber bolt as the main anchor. You can hike up to the top of the rock and rappel off about 30' with part of it being over-hanging. It's perplexing how 3 people could have fallen at the same time if they were on rope or anchored in, but it's hard to tell from the info provided by the news what exactly happened. My thoughts go out to the families involved in this tragedy.

I think Rescue rock is also called Sugar Cube boulder, possibly the same as Ice cube?
I also read in the news that one of the injured kids passed away from his injuries as well.

The FD was calling it the Sugar Cube on the radio while the rescue was going on.
